Victoria tooth extraction and implant, timing decided early

If a tooth is coming out and an implant is even a possibility, plan the tooth extraction and implant together rather than months apart. What is done on that day decides how much bone is there later.

Three routes

At extraction: post today, graft today, or nothing

Where the socket is clean and the walls intact, a post can go straight into it, which saves months and avoids a second surgical visit. That is covered on Same Day Implants and it is not available for every tooth.

Where it is not, filling the socket with graft material at the same appointment keeps most of the ridge shape that would otherwise be lost. That is Socket Preservation, a small addition to an appointment you are already having.

And doing neither at the extraction is a legitimate choice, if you are certain no implant will ever be wanted there. What it costs you is the option, because rebuilding a collapsed ridge later means Ridge Augmentation.

How it is decided

Deciding at the extraction appointment, in four steps

  1. Is there infection?

    Active infection usually rules out placing a post the same day, because a fresh implant needs a clean site to integrate.

  2. Are the socket walls intact?

    A post needs bone around it. A blown out cheek side wall means grafting first rather than placing into a gap.

  3. Can the tooth come out gently?

    Sectioning a root often preserves more bone than forcing it. How the tooth is removed matters as much as what follows.

  4. Do you want an implant at all?

    If yes or maybe, preserving the socket is cheap insurance. If genuinely never, it is money you do not need to spend.

Why timing matters

Most shrinkage after a tooth extraction happens within six to twelve months, and the cheek side wall loses the most. A gap left for a decade is a fundamentally different case from one left for a season, which is why the same tooth in two different patients can produce two very different quotes. Sometimes. It needs a clean socket with intact walls and enough bone beyond the root tip to hold the post firmly. Infection, a fractured wall or soft bone all point toward waiting. The scan and the state of the tooth decide, not preference.

No. Once the socket has closed, the opportunity has gone and any rebuilding becomes a separate augmentation procedure. That is the single most important thing to understand: the decision belongs to the extraction appointment.
Tell them you are considering an implant and ask them to preserve the socket. Most will, and it is a routine addition. It is far better to raise it with whoever is doing the extraction than to arrive here months later wishing it had been done.
